Graceful Passages, The Hospice Journals offers an
inside look into the poignant world of Hospice care as seen
through the eyes of caregivers, patients and staff.
The 60-minute film was created by Jon R. Hand, an independent
filmmaker who spent over two years with Hospice patients and
staff. Graceful Passages follows several patients and their
families into hospitals, homes, assisted living centers, and
Hospice inpatient centers.
