PROLOGUE
Often, hospice patients need the acute management of their symptoms in a Hospice Inpatient Unit, or if not available, in a nearby Hospice bed in the hospital. Here, one patient discovers the inevitable is not so close in coming.
               
 
HOSPICE TEAMWORK
One of the unique features of Hospice care is the coordinated, professional team that makes regular patient visits and meets every week on each Hospice patient. The interdisciplinary team is the dependable support system of the patient and family.
 
 
MILTON’S SONG
Hospice’s Assisted Living and Nursing Home team partners with long term care staff to enable residents with serious illness to remain in the comfortable, safe setting of their residence, and reduce the need for hospitalization at end of life. The resident in this journal had congestive heart failure and received 18 months of hospice care.
 
 
DISCOVERY
Education of tomorrow’s doctors and health professionals regarding palliative and hospice care is crucial to achieving improved quality of life for those at the end of life. Currently, too many hospice patients are referred very late by their physician and do not benefit from the intensive support hospice care can provide.
 
 
DR. MARY
The Hospice physician is integral to the Hospice Inpatient Unit setting where patients and their families come for acute management of difficult symptoms, respite care, and end-of-life care.
 
 
MARJORIE and DAVE
The in-depth and compassionate care offered by Hospice in the home is illustrated through the loving eyes of this caregiver and spouse. It is a tribute to the family caregiver and how Hospice can assist in helping them to maintain the patient at home despite advancing disease.
 
 
EPILOGUE, THE HERNANDEZ FAMILY
Helping families and their loved ones at one of the most difficult times in their lives is a privilege. Here we see how a family can say goodbye in their own unique way while honoring the matriarch (the mother) that has brought them all together.
