Hospice’s Assisted Living and Nursing Home team operates in more than 40 facilities, partnering with long term care staff to enable residents with serious illness to remain in the comfortable, safe setting of their residence, and reduce the need for hospitalization at end of life. The resident in this journal had congestive heart failure and received 18 months of hospice care.
